How to order a CSDC publication:

Download and complete a Publications and Merchandise Order Form. Send to CSDC with payment (check or purchase order)
Upon receipt of order form and payment, CSDC will e-mail a licensing agreement
Read and sign licensing agreement. Return to CSDC via fax [ (916) 471-0155 ] or mail
Upon receipt of signed licensing agreement, CSDC will ship the publication(s).

CSDC grants to the original purchaser a limited, non-exclusive license to use the materials, documents, templates and samples created by CSDC and provided to the original purchaser for the development, operation and supervision of one charter school. These materials, documents, templates and samples may not be duplicated or otherwise shared without the express written consent of CSDC.
